If you are planning on trying out for a sport or cheerleading at Center Grove High School, you must have the following items completed:
a completed physical on file, complete with IHSAA Physical Form, St. Francis Emergency Card and a DAT Consent form, if you are an underclassmen.
A Transfer Report form must be completed with the Assistant Athletic Director if you have transferred from another school. You can find this form on this website under "Transferring to Center Grove"
We strongly encourange all athletes to be in good standing academically.
