2021-2022 ALL SPORTS PASSES
Support your Trojans by purchasing the 2021-2022 All Sports Pass.The All Sports Pass admits buyers into all regular season, home-only Varsity, JV, & Freshmen events (not valid for Johnson County or IHSAA post-season events). Student and adult passes remain $50, Senior Citizens (62 and older) remain $40. All Sports Passes will be electronically redeemed via event workers.
If you see positive sportsmanship on display from any school, please use the link below to report it. A positive report gives a school one sportsmanship point with the IHSAA. FORMS & POLICIES
The CGHS Athletic Department is implementing new Physical documentation procedures for the 2021-2022 school year.
Forms must be completed electronically at
https://www.rankonesport.com/content/ (Tutorials are available)
There are 3 forms to complete online. Parents/students will need to know Student ID number to complete forms online.:
1. Concussion and Sudden Cardiac Arrest Acknowledgment
** Must first read “Heads Up Concussion” Fact Sheet for Athletes and Parents
** Must first read “Sudden Cardiac Arrest” Fact Sheet for Athletes and Parents.
2. Consent to Treat (contains emergency contact info) &
3. HIPAA Authorization Form
PLEASE PRINT and COMPLETE THE ENTIRE IHSAA Preparticipation Physical Form and RETURN to the Athletic Office prior to participation in any IHSAA sport for the 2021 - 2022 season. Week 1 begins July 5, 2021.
All other forms remain electronically filed.
*FRESHMEN AND TRANSFER STUDENTS MUST COMPLETE AN RDT FORM. (see forms below)
