Transition Resources
Transition to adulthood information that is required by students and parents can vary each year depending on the wants and needs of the student. Every student is different and will require different transition to adulthood information at different times. The following are documents and web pages that would be appropriate for students and parents in the transition process.

Many parents of students in transition are facing a steep learning curve and have questions about Social Security and Medicaid Waivers and about how employment income could impact their child’s long-term supports.Using some of the questions she received in her presentations, CCLC has developed “Working and Benefits,” a series of short videos featuring Ms. Lott answering some of the most commonly asked questions. The series includes:

Transition Assessment Matrix
The Transition Assessment Matrix was developed for secondary education transition teachers by the Indiana Northeast Cadre of Transition Leaders and the Indiana Secondary Transition Resource Center (INSTRC). Assessments included in the Matrix have either been created by INSTRC, are available online, or are those for which INSTRC has received permission from the original publisher.

Transition planning is a different kind of planning. It is planning for adult life through school and non-school experiences. The ideal outcome of good transition planning is the uninterrupted movement to adult life. This may include post-secondary education or training and/or community employment, community living and an integrated adult life, including leisure opportunities with family and friends.
