• No cost for doctor visits or lab services.

Vision Insurance

• Vision Insurance option available through Anthem
• Eye exam - Once every 12 months with $0 copay
• One pair of eyeglass frames - Once every 24 months
• Eyeglass lenses - Once every 12 months, $10 copay
• Contact lenses - Annual allowance

Dental Insurance

• Dental Insurance option available through Delta Dental
• Oral exams/cleanings - 2 per year
• Total benefit - $2,000 per person, per year
• Orthodontic services - $1,000 per person, per lifetime

The Center Grove Wellness Center is available to Center Grove employees and their families who are on Center Grove's health insurance plan. The Center provides to eligible employees many services and care at no cost, including physicals, screenings, health education, treatment for minor injuries and illnesses, and more.
